A pneumopericardium caused by gastric ulcer perforation.

چکیده
A 72-year-old man with a history of chemoradiation therapy for lung cancer and with known gastric ulcer was admitted due to chest pain. On day 6, a chest X-ray showed air around the heart (Picture 1) and computed tomography showed a fistula between the pericardium and the gastric ulcer (Picture 2). A diagnosis of cardiac tamponade caused by a pneumopericardium was made, and a drainage tube was inserted into the pericardium (Picture 3). Although the fistula had closed, later it was found to be patent. The patient ultimately died due to heart failure. Pneumopericardium, the presence of air in the pericardial space, was first described by Bricketeau (1). This rare condition is caused by trauma, positive pressure mechanical ventilation (2) and esophagopericardial fistulas. In our case, the patient’s past history of radiation therapy may have compromised the strength of the pericardium and gastric wall, leading to the development of an intractable fistula and pneumopericardium although the direct cause was a gastric ulcer.

A 88-year-old male patient presented with fever, singultus and retrosternal pain. After 8 days of antibiotic therapy not resulting in clinical improvement, he suddenly developed a pneumopericardium. Contrast swallow and endoscopy showed intrapericardial perforation of a benign gastric ulcer. Excision of the ulcer and suturing of both the stomach and the diaphragm as well as lavage of the perica...
متن کاملPenetration of the pericardium by a gastric ulcer--survival after pericardiocentesis.
Elderly patients often have unusual manifestations of common illnesses. We describe a geriatric patient having a pneumopericardium from pericardial penetration by a gastric ulcer. Benign gastric ulceration causing perforation of the pericardium or other cardiac structures was recognized as early as 1854; however, until 1964, the condition was invariably fatal. Our patient survived the episode a...

A 57-year-old man, who had undergone subtotal esophagectomy and bowel reconstruction with a stomach roll located in the retrosternal space 4 years before, developed pneumopericardium (Fig. 1) due to ulcer perforation (Fig. 2). He is alive and well 8 years after partial gastrectomy and pericardial drainage followed by pericardiectomy for secondary constrictive pericarditis. European Journal of C...

The presence of free air within a hiatal hernia sac is an uncommon reported entity. This finding has been described in the surgical literature1-4 secondary to incarcerated hiatal hernias complicated by gastric ulceration and perforation. There have, however, been no reports of this entity in the radiology literature. Our patient presented to the emergency department complaining of epigastric pa...

A 57-year-old woman with a medical history of hiatal hernia, peptic ulcer disease, alcoholism, and schizophrenia was admitted to the emergency department with epigastric pain and fever. The patient had not been taking her prescribed proton pump inhibitors. A peptic ulcer perforation (1.5cm) in the hiatal hernia sac was discovered during gastroscopy, and an attempt to close the perforation with ...
